COVID Players Agreement

All players aged over 18 must complete this agreement before they can train.

You only need to fill this in once.

Introduction

This is an agreement between you (a player) and Maidstone United Raiders during the COVID-19 pandemic.

You must read and sign this before you can attend a Raiders training session.

No one will be allowed to attend training unless this form is signed.

You are choosing to attend the training session and you understand that all coaches will follow all guidelines but we cannot provide any guarantee with regards to you contracting COVID-19. *

You will only attend training sessions when you feel well and, by attending the training session, you are confirming that you do not have COVID-19 symptoms. *

You will not attend training sessions for 14 days if you have been in contact with someone who have COVID-19. *

You must always remain 2 metres apart from all other parents and guardians in attendance, and other players when not participating in a coach-led training session. *

For every training session, you will bring your own water bottle and hand sanitiser. *

You will maintain the 2m social distancing when arriving and leaving the venue. *
